From the Dallin Nurse: Vaccine clinics and health screenings

Families,
I hope that you and your family are having a wonderful back to school September. I am new to Dallin this year and I look forward to meeting all the new Dallin Elementary School faces. I want to remind you that Dallin has a vaccine clinic coming up in October. We also have school health screenings that start in the Fall.

 

Vaccine Clinic:
We are hosting a Vaccine Clinic in the Gym on Monday, October 21 from 3pm-6pm. COVID, Flu and other vaccines will be available. Please look here for more information! Ages 3 and up, everyone is welcome. Bring your insurance information. Sign up here or just show up! If you’d like to fill out the consent form beforehand, click here for a copy.

 

School-based Screenings:
Vision and Hearing Screenings will begin soon, starting with Kindergarten. Kindergarten through 3rd grade vision testing includes distance and near acuity, as well as stereopsis, while vision testing in 4th and 5th comprises distance only. Hearing is tested in grades Kindergarten through 3rd grade. You will be contacted if your child does not pass any of the screenings. Other screenings that are mandated for school children are height and weight for grades 1 and 4 (which can mostly be collected from current physicals), and scoliosis screening for 5th grade, which fifth grade parents will have to opt-in and sign consent for if they would like their child screened - prior notice and consent form will be emailed in April 2025.
