End of Year Message from APS Nurses

Medications at School:
- Per APS policy, parents/guardians or a responsible adult shall pick up their student’s medication in the nurse’s office by the last day of school. Any medication not picked up by the end of the day on June 25 will be discarded (note: this is an 11:30 am release).
- If you would like the school nurse to send home your student's Epipen or inhaler in their backpack on the last day of school, please let us know. All other medications must be picked up by a parent/guardian, or another trusted adult.
- If your student will take a daily prescription medication during school hours for the 2026-27 school year, be sure to have their prescriber complete a new Medication Order Form.
Physical Exams:
- If your student will be in kindergarten or 4th grade for the 2026-27 school year, please submit their current physical exam (from within the last 12 months), making sure that the form is signed and dated by the health care provider.
- All incoming kindergarten students must have a physical exam & up-to-date immunization record on file before starting school.
- If your student is not entering kindergarten or 4th grade, and you would like to still provide a copy of their most recent physical exam, we will happily accept!
Life-Threatening Allergies, Asthma, Diabetes, Seizure Disorders:
- If your student has one of these diagnoses, please make sure to submit updated medical paperwork and medication to their school nurse, no later than the first day of school in September.
- All forms may be found on the APS Health & Nursing Services website.
If your student has any other significant change in their medical history over the summer, please be sure to let their school nurse know before the first day of school so that we can plan accordingly.
During the summer, you may send updated medical documents via the SNAP Health Portal or by mail. Please note that these will be processed when staff return on August 31.
